JOB TITLE: Executive Director

POSITION: Salaried Position

REPORTS TO: Gateway Home Board of Directors

POSITION DEFINITION: A Team leader responsible for overseeing the day to day operation of the Home who strives to incorporate the philosophy and mission of Gateway Home by consistently exemplifying the values of compass ion, hospitality, teamwork and a commitment to excellence.

QUALIFICATIONS:
• Works as an equal part of the team for the betterment and continuation of the mission of Gateway Home.
• Have experience in a supervisory capacity for 3 years or more.
• Possess excellent organizational, written and oral communication skills.
• Ability to work in a changing health care environment. • Possess the ability to develop and build relationships with the community, Gateway Home staff and volunteers.
• Ability to work irregular hours on occasion.
• Grant writing experience preferred.

DUTIES AND RESPONSIBILITIES: The following statements reflect the general duties considered necessary to describe the principal functions of the job as identified and shall not be considered as a detailed description of all work requirements that may be inherent in the position.
• Provide leadership to the staff and volunteers while setting realistic goals and deadlines and developing and maintaining quality healthcare services, delivered in a fiscally responsible manner consistent with the values of Gateway Home.
• Ensure maximum utilization of resources by assisting in the preparation of budgets, allocation of funds, grant writing and overseeing accounts payables ensuring fiscal stability.
• Expenditures, purchases of major items and other financial matters of Gateway Home exceeding $1,000 must have prior approval of the Board of Directors.
• Serve as Gateway Home representative at meetings of professional, civic and service organizat ions in collaboration with the Director of Development, other staff, volunteers and the Board of Directors.
• Submit a monthly written report of staff, volunteer and resident care overall operations during the previous month to the Board of Directors.
• Attend Board of Directors meetings as a non-voting member and participates in Committee meetings as deemed necessary.
• Participate with the HR Committee in hiring on the basis of positions established by the Board of Directors. • Conduct 90-day performance evaluations for paid staff, as well as annual and any additional performance evaluations thereafter ensuring that job descriptions are being fulfilled. Performance evaluations for Gateway Home employees need to be documented and completed in a timely manner.
• Conduct a minimum of six (6) staff meetings to include all paid employees per year.
• Review and monitor duty assignments, time schedules, work hours and duty assignments of all personnel and volunteers in collaboration with other staff and approve payroll for processing.
• Takes disciplinary action with personnel when deemed necessary after documenting appropriately and communicating with the HR Committee
• Keep abreast of community activities and legislation affecting the healthcare industry.
• Assist with fundraisers as appropriate in collaboration with the Director of Development.
• Responsible for all aspects of the Annual Campaign and the Annual Memorial Service and signing Thank you’ s to all donors.
• Assist with recruiting, interviewing, training and mentoring volunteers ensuring the volunteer/staff data base is updated monthly and accessible to all staff and volunteers.
• Be a supportive presence in the Home while ensuring adequate coverage in collaboration with volunteer schedulers.
• Responsible for organizing and implementing regular team building events for volunteers and staff such as recognition luncheons, retreats, picnics, etc.
• Responsible for co-chairing special family & resident events such as the Annual Memorial Service. • Provide support to residents and families including attendance at wakes and funerals when possible.

TITLE: Office Manager

POSITION: 30 hours per week

REPORTS TO: Executive Director

POSITION DEFINITION: Responsible for organizing and coordinating office operations and procedures in order to ensure organizational effectiveness and efficiency. Manage the financial and accounting operations of Gateway Home. Provide various bookkeeping and accounting services, including accounts payable, accounts receivable, payroll, inventory and cash receipts.

QUALIFICATIONS:
• Works as an equal part of the team for the betterment and continuation of the mission of Gateway Home.
• Minimum of two years office experience.
• Knowledge of Microsoft Word, Excel and Database programs
• Possess organizational skills and the ability to prioritize workload
• Possess excellent verbal and written communication skills
• Possess basic bookkeeping skills.

DUTIES AND RESPONSIBILITIES: The following statements reflect the general duties considered necessary to describe the principal functions of the job as identified and shall not be considered as a detailed description of all the work requirements that may be inherent to the position.
• Documents daily activities on timesheet.
• Supervises trains, assigns duties and schedules office volunteers.
• Maintains volunteer / staff bulletin board to promote effective communication.
• Maintains & updates volunteer personnel files and all databases.
• Operates and maintains office equipment and notifies the Executive  Director of maintenance needs.
• Maintains an inventory of office supplies and orders supplies as needed notifying the Executive Director. Promotes cost containment within the annual budget.
• Maintains up-to-date originals forms and ensures copies and printed materials are readily available to staff and volunteers.
• Maintains office files in an orderly manner, easily accessible to key office staff.
• Picks up, opens and sorts mail in a timely manner.
• Facilitates bulk mailings and ensures outgoing mail is delivered to the post office in a timely manner.
• Maintains computer databases and makes regularly scheduled backups.
• Facilitates thank you letters and acknowledgements to donors of money, in-kind donations and donated services in a timely manner. • Attend regularly scheduled staff meetings of all paid staff.
• Assists Board of Directors, staff and committee chairs upon request.
• Prepares payroll forms for the payroll service, along with all supporting time sheets, and appropriate paperwork and submits such to director for approval.
• Maintains a complete and systematic record of business transactions utilizing financial accounting software including Quick Books (accounts payable: writing and mailing checks) and donor tracking software including donor express, (recording accounts receivable). Manage petty cash and gift certificates. Responsible for counting cash, checks, making bank deposits and recording such with proper documentation in a timely manner.
• Assures accuracy of all financial records at all times, making such available to Executive Director / Board of Directors on request.
• Maintains personnel files and notifies appropriate supervisors of compliance needs of all paid staff in a timely manner.
• Perform additional tasks as deemed necessary by the Board of Directors and Executive Director.

TITLE: Director of Resident Care

POSITION: Twenty-five (25) hours per week

REPORTS TO: Executive Director

POSITION DEFINITION: Directly responsible to develop a comprehensive end­ of-life plan of care addressing the physical, emotional, spiritual and social needs of residents and family members while affirming a dignified quality of life. To supervise volunteers and staff members.

QUALIFICATIONS:
• Nurse currently licensed in New York State
• Minimum of two years clinical nursing experience, including end-of-life care
• Preferably with some supervisory experience
• Demonstrates effective leadership/management skills and ability to function as a team leader

• Serves as a liaison and good will ambassador between Gateway Home and hospital personnel, physicians and their office personnel, certified agencies and the faith based community.
• Conduct interviews with job applicants for staff nurse and health care aide positions to determine applicant’s desirability as an employee.
• Collaborates with the Executive Director and other key staff members as well as the Human Resource committee prior to hiring new employees.
• Schedules staff to provide adequate coverage, including the on-call component. Responsible to find replacement staff/volunteer should a cancellation occur during his/her shift
• Takes disciplinary action with staff and volunteers as warranted after reporting to Executive Director. May collaborate with Human Resource committee if deemed necessary.
• Prepares a monthly report and submits it to the Board of Directors.
• Schedules, prepares agenda and facilitates staff meetings.
• Participates in the revision and development of policies, procedures and documentation relating to resident care. Is responsible to inform and educate staff and volunteers regarding any changes made.
• Coordinates and facilitates resident transfers, resident care, and family conferences.
• Collaborates with referring entities and certified agencies following the case, utilizing effective communication between team members to achieve maximum symptom control management.
• Reports to the certified agency with status changes and gives suggestions regarding comfort care measures.
• Ensures the security, availability and accurate distribution of medications for the residents.
• Ensures adequate medical supplies and durable medical equipment for residents.
• Reviews, evaluates and documents services and care rendered.
• Mentors, supports and supervises volunteers to ensure continuity of care and to provide supportive and safe environment in conjunction with the Manager of Volunteer Services and Executive Director.
• Performs additional tasks as deemed necessary.
